Your Discharge

Your physician will decide when it is appropriate for you to leave the hospital and will write a discharge order. Please arrange for a family member or friend to assist in your departure. Your physician or nurse will provide post-hospital care instructions. Should you have any questions about diet, activities or other matters, ask at this time.
 
For your safety, we will provide wheelchair transportation to the lobby exit where a family member can safely pick you up in the car.  Be sure to gather all of your personal belongings and ask for any valuables you may have in our safe.

After Discharge

Quality medical care provided to you in a friendly atmosphere is our primary goal. Because we value your opinion of our services, we would appreciate your rating the care you received. As a patient, you are the one who can tell us if we have been successful.  

If you experienced care in our Emergency Department, Ambulatory Surgery Center, as an Inpatient or with Outpatient Testing, you may receive a random telephone satisfaction survey call from the Gallup Organization and/or a mailed survey from the Centers for Medicare & Medicaid Services (referred to as the H-CAHPS survey). The confidential feedback that you provide will help guide future enhancements to our facility.
